Encore Rehabilitation is excited to announce the upcoming launch of eFit at Home, set to launch on December 1st, 2022, in a phased roll-out plan. eFIT at Home will provide Functional Individualized Therapy (FIT) to seniors enabling them to live independently in the comfort of their homes, wherever home may be. Our aim is to offer residents hope through a personalized understanding of their mind, body, and purpose with resident-driven care to improve their overall wellness.
While we have other service lines that service varying age groups, our care for seniors has been most prevalent largely in part to statistics on the aging population. The population is aging rapidly, and we are approaching a pivotal time for our seniors as many are making choices about their desires for future living arrangements.

Aging in place has stayed the largely preferred model, with more than 77% of adults 50 and older expressing the desire to remain at home long term. While aging in place is preferred, it may not be feasible for all due to varying reasons like lack of resources.
As one of the nation’s top contract therapy providers for the skilled nursing setting, we have seen the continual shift to non-institutional care and the emphasis on ensuring our seniors have access to post-hospitalization resources to allow for successful aging in place. This shift was particularly highlighted with the Choose Home Care Act 2021, S. 2562/H.R. 5514, aimed at providing seniors the choice to transition home after a hospitalization in lieu of a skilled nursing stay by increasing the resources available to them.
With a larger portion of the senior population bypassing the skilled nursing setting and returning home for extended care services, a need within the continuum has been highlighted to ensure successful aging in place. There is a cyclical pattern that exists within the system that is created when a patient no longer qualifies for home health care services and is left to maintain their current level of function until what seems inevitably to be the next fall, acute illness episode, or decline in function. We are looking to bridge that gap with eFit at Home by providing therapy services that support overall wellness that utilizes a unique, holistic approach to put the patient at the center of their care.

We are excited about the launch of eFIT at Home. It will allow us to continually serve our senior population in an enhanced capacity, focusing on their desires to remain in the home environment and community for as long as possible. We aim to continue fostering more dynamic relationships with our seniors and expanding our presence within the continuum to allow us to be the provider of choice across settings.
