eFIT at Home provides Functional Individualized Therapy (FIT) to assist seniors in successfully aging in place. We are focused on care at home, wherever home may be, while forming relationships with patients across the continuum of care.

The goal of eFIT at Home is to become the provider of choice to support seniors’ desire to remain at home and bridge the gap in the continuum to provide a proactive approach to wellness with a Mind, Body, Purpose approach to improve health from a holistic perspective.

We believe a patient’s health is affected by his or her mental, physical, and spiritual states. When we take care of all three, the patient can enjoy life at home and in the community safely, independently, and comfortably. Our unique Mind, Body, Purpose approach was created with this in mind and encourages our therapists to develop patient-centered treatment plans aimed to improve each patient’s health from a holistic perspective.



Our GeroPsych service works to help residents cope and understand illnesses that affect them emotionally and physically through performing cognitive assessments, conducting therapy sessions, and consulting families. Our Geropsych program aims to help our residents maintain well-being, overcome problems, and achieve maximum potential in later life.


We address cognitive-linguistic deficits through therapy services that address the cognitive abilities that start to decline as we age. By handling executive function tasks, we can help residents manage time, remain organized, form goals, plan and execute plans effectively and self-monitor. Our therapists work as a team to address executive function impairments. These impairments not only affect memory but also correlate with residents with balance and gait impairments who rely on cognition to compensate for physical deficits.


Pain Management

Pain can make it hard to do the things residents enjoy and can change their relationships and independence, as well as cause physical symptoms. Encore therapists work with residents to restore normal movement patterns and offer compensatory techniques when necessary. We also offer modalities such as myofascial release, soft tissue manipulation, and knowledge of modalities to facilitate pain management with musculoskeletal injuries.

Swallowing and Diet Analysis

For residents whose quality of life has been affected by difficulties during mealtimes, our therapists can help provide a dietary analysis to determine if a resident has a potential need for therapy to address this problem. Through diet alterations, pacing, postural/position techniques, and maneuvers, our therapists can help provide relief that helps make eating and/or drinking more comfortable and safe.

Balance and Falls

Our therapists will consider many causes of balance changes when developing a service plan to help residents regain muscle strength and balance, improving their gait quality and reducing their fall risk. We also offer a virtual reality (VR) solution for assessing fall risk in seniors and other populations, which allows our clinicians to identify risk factors and focus on prevention.

Incontinence Prevention

Incontinence can harm a resident’s social, emotional, and psychosocial life. Our clinicians use pelvic muscle exercises, environmental adaptations, and bladder retraining to help residents improve their lives by reducing incontinence episodes.


Our wellness program encourages residents to adopt healthy habits and engage in activities that promote better health and improve their quality of life. We provide various wellness services based on the whole person concept, which recognizes that everything a resident does, thinks, feels, or believes impacts their health.

To promote wellness,  we offer remote therapeutic monitoring devices to track, assess, and monitor residents’ health conditions, musculoskeletal system, respiratory system, therapy adherence, and response.


Asking Quality Questions

Our therapists ask all the (right/meaningful) questions to identify:

  • A baseline of prior level of activities of choice
  • Presenting underlying impairments
  • Previous and current activities that motivate an individual
  • Areas of interest that can be incorporated into treatments
  • Areas that will fold into the RESTORE – COMPENSATE – ADAPT (RCA) Model

How eFIT at Home Works



Our wellness program offers our patients access to over 140 full-length exercise and meditation sessions through our partner Spiro100. This includes web and app streaming fitness instructions, creating a mobile workout that you can take with you anywhere. These are produced and led by nationally recognized senior wellness experts to provide you a purposeful, results-driven fitness tool.

Lightyear Health

With Lightyear Heath’s help, our GeroPsych service offers physician-led physical medicine & rehab (PM&R) and behavioral health (BH) services for residents in senior living communities with onsite APPs, MD support through telemedicine and all services provided at no cost to you.

Upright VR

We have partnered with Upright VR to test and challenge the postural control system, which allows for sensitive and specific identification of the causes of balance deficits. This approach is used to track recovery and can then be used to help and prescribe treatment or guide preventative training regimens.

BCAT® Cognitive Approach

Targeting cognitive and functional deficits may accelerate rehabilitation outcomes. We examine the efficacy of using working memory exercises (WME) in a prescriptive treatment approach for patients with MCI and dementia (mild to moderate), applied concurrently with rehabilitation services, for improving activities of daily living (ADL).
